Technicians write arrival time, treatment of areas and target pests and notes the type of service completed.
Chemicals will be printed and noted in the blank area so you know what we treated at your home at every appointment.
Billing history and past due amounts will appear in the middle lower right white box.
Customers can write and send in questions or concerns in the remittance copy.
